And we do it all with heart, each and every day.**Position Summary**Population Health management is built on a detailed understanding of the distribution of social, economic, familial, cultural, and physical environment factors which impact health outcomes among different geographic locations and groups (such as socioeconomic, racial/ethnic, or age), and the distribution of health conditions, health related behaviors and outcomes including but not limited to physical, dental, mental, and Health Related Social Needs among different geographic locations and groups (such as socioeconomic, racial/ethnic, or age).This is a fully remote position. Eligible candidates must reside in Michigan.**Core responsibilities*** Responsible for all population health management components.* Oversee the strategic design, implementation, and evaluation of population health initiatives based on a deep understanding of scientific population health principles.* Sponsor and champion system-wide initiatives, including cultivating the support necessary to achieve the desired objectives of supporting equitable whole person care for Enrollees.* Develop and implement operational plans that address the market opportunities/challenges and align with the established population health goals of serving the whole person, including Enrollees’ medical and non-medical needs.* Must coordinate with the Medical Director and the Quality Improvement and Utilization Director and external parties on population health activities.**Required Qualifications*** 10 years’ clinical and/or managed care experience in the healthcare industry* Must reside in Michigan* 7 years’ Experience with Medicaid managed care or Population Health* Active and unrestricted registered nursing license or social worker license in state of residence.* Mastery of problem solving and decision-making skills* Mastery of collaboration and teamwork* Mastery of growth mindset (agility and developing yourself and others) skills* Mastery of execution and delivery (planning, delivering, and supporting) skills* Mastery of business intelligence**Preferred Qualifications*** Previous experience that reflects a proven track record or proficiency in the competencies noted.* Previous experience with Managed Care* Demonstrated verbal/written communication and computer skills.* Ability to work in a fast-paced environment.* Demonstrated ability to plan, organize, and execute multiple functional business objectives.* Exceptional communications skills including ability to interact with a variety of external and internal audiences including senior management.* Certified Professional in Health Quality (CPHQ) or other certification a plus.* Proficient in Microsoft Office Suite, including Word, Excel, Outlook, OneNote, and Teams, with the ability to effectively utilize these tools within the context of the Population Health Director role* Confidence working at home / independent thinker, using tools to collaborate and connect with teams virtually**Education**Master’s degree or other advanced degree in nursing, social work, health services research, health policy, information technology, or other relevant field**Pay Range**The typical pay range for this role is:$110,467.00 - $237,930.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
